What HOMA-IR is
HOMA-IR (Homeostatic Model Assessment of Insulin Resistance) is a mathematical model that estimates insulin resistance, a state in which cells respond less to insulin so the pancreas has to release more of it for the same job. The model was created so resistance could be estimated from an ordinary blood test, without the complex procedures used in research.
It matters to understand what HOMA-IR is and is not. It is an estimate, not a measurement. It captures a single morning, on an empty stomach, and it only means something alongside your symptoms, body weight, waist circumference, family history and other results. That is why the HOMA index is never read on its own; an endocrinologist reads it together with the whole picture.
How the HOMA index is calculated
When glucose is reported in mmol/L and insulin in mIU/L (the same as µIU/mL), which is standard in laboratories in Serbia, the formula is:
HOMA-IR = (fasting glucose × fasting insulin) / 22.5
If your glucose is given in mg/dL, the divisor is 405 instead of 22.5. Example: glucose 5.2 mmol/L and insulin 12 mIU/L give a HOMA-IR of (5.2 × 12) / 22.5 = 2.77.

What HOMA index values mean
Reference values differ between laboratories, because they depend on the method used to measure insulin. Always check the reference range printed on your own report first. As a guide, these ranges are the ones most often used in adults:

Two situations are often missed. First: HOMA-IR can look normal while fasting insulin is high, because the body is still holding glucose steady at the cost of releasing more insulin. Second: once diabetes has developed and the pancreas produces less insulin, HOMA-IR loses reliability and is not used for monitoring. In both cases the number alone misleads, and the clinical picture does not.
How to have blood drawn for a HOMA index
The model assumes a fasting state, so values measured after a meal cannot be entered into the formula. For the result to be usable:
- Blood is drawn in the morning, after 8 to 12 hours without food. Water is allowed.
- Avoid a large or very sweet meal the evening before, and avoid alcohol.
- Avoid intense training the day before, as it temporarily changes insulin sensitivity.
- If you take medication that affects blood sugar, or corticosteroids, say so, because it changes the interpretation.
- Women do not need to wait for a particular day of the cycle for a HOMA index.
If the result is borderline, an endocrinologist often adds an OGTT with insulin levels, a glucose tolerance test that tracks both sugar and insulin over two hours. That test shows what HOMA-IR cannot see: how your body reacts to a meal, not only how it sits while fasting.
A raised HOMA-IR: what next
A raised HOMA index is not a reason to panic, but it is a reason to act, because insulin resistance is easiest to reverse early. For most people the first and most effective step is not a medicine but a change of routine: meal timing, fewer fast carbohydrates, regular physical activity and, where needed, a moderate reduction in body weight. Even a modest weight loss improves insulin sensitivity substantially.
An endocrinologist assesses whether medication is also needed and whether something else sits behind the resistance, most often a thyroid disorder or polycystic ovaries.
