HbA1c Calculator: Average Blood Sugar and What Your Value Means
HbA1c, glycated haemoglobin or the "three-month sugar", is one number that says more than dozens of glucometer readings: it shows what the average blood sugar was over the previous two to three months. The calculator below converts HbA1c into average glycaemia in mmol/L and mg/dL, switches percent to mmol/mol and back, and says whether the value is normal, prediabetes or diabetes. Below it are interpreters for fasting glucose and the oral glucose tolerance test (OGTT), because the diagnosis is usually made from the combination of those three results.

What HbA1c is and why it is "three-month"
Glucose in the blood binds spontaneously to the haemoglobin in red blood cells. The higher the sugar and the longer it lasts, the larger the share of haemoglobin that is "sugared". A red cell lives about 120 days, so HbA1c reflects the average of the previous two to three months, with the last month carrying the most weight. That is why one sweet the day before the blood test cannot spoil it, but a week of dieting before a check-up cannot fix it either.
Average glycaemia (eAG) is calculated from HbA1c with the formula from the ADAG study: eAG (mmol/L) = 1.59 × HbA1c − 2.59, or eAG (mg/dL) = 28.7 × HbA1c − 46.7. A value of 6.0 % thus corresponds to an average sugar of about 7 mmol/L, which is more than most people expect when they see "only six". Laboratories increasingly report the IFCC unit mmol/mol alongside percent; 6.5 % is 48 mmol/mol.

When HbA1c misleads
HbA1c assumes a normal red cell lifespan. Anything that shortens or lengthens it shifts the result, and the doctor then works with glucose and the OGTT:
- iron deficiency anaemia falsely raises HbA1c, and iron replacement lowers it
- blood loss, haemolysis and a recent transfusion falsely lower it
- pregnancy, especially the second and third trimester, lowers it; in pregnancy diabetes is diagnosed with the OGTT
- kidney and liver disease and some haemoglobinopathies change glucose binding
- in children and young people with sudden symptoms HbA1c lags behind the real state

Prediabetes: what is done when the value is 5.7 to 6.4 %

What is a normal HbA1c value?
Below 5.7 % (below 39 mmol/mol). From 5.7 to 6.4 % is prediabetes, and 6.5 % or more corresponds to diabetes if confirmed by a second measurement. In people who already have diabetes the target for most is below 7.0 %.
What is the average sugar at an HbA1c of 6 %?
About 7.0 mmol/L, or 126 mg/dL. It is calculated with the formula eAG (mmol/L) = 1.59 × HbA1c − 2.59. Every 0.5 % of HbA1c corresponds to a change in average glycaemia of about 0.8 mmol/L.
Do you have to fast for HbA1c?
No. HbA1c does not depend on the last meal, so it can be taken at any time of day. You fast when glucose, insulin or lipids are done on the same sample.
How often is HbA1c checked?
In prediabetes and stable diabetes every 6 months, with a change of treatment or poor control every 3 months. More often than that makes no sense, because the red cells need time to renew.
What is the difference between HbA1c and glucometer glucose?
A glucometer measures the current sugar, which changes from hour to hour, and HbA1c the average over two to three months. A normal morning glucometer reading does not rule out prediabetes, because sugar often rises first after meals, which HbA1c and the OGTT see and a morning measurement does not.
