Eyelid Surgery (Blepharoplasty): Cost, Recovery and When It Is Not the Answer
Eyelid surgery removes the excess skin and the fat pads that give a face its tired look. What it does not do is fix everything you can see around your eyes. A good share of the people who come in about "drooping eyelids" actually have a low brow position or dark under-eye shadows, and those are different conditions with different treatments. This article sets out what blepharoplasty really changes, how recovery runs day by day, what it costs in Belgrade and when something else is the smarter choice.
What blepharoplasty fixes, and what it does not
This is the most useful part of the article, because most disappointment after facial surgery starts right here: the operation went well, but it was not working on the thing that actually bothered the patient.

There is a simple check you can do at the mirror before you book anything. Lift your eyebrow a few millimetres with a finger. If the heaviness disappears and the eye opens up, the brow position is the main culprit. If the fold of skin still lies on the lid, you are looking at excess skin and upper eyelid surgery is the right address.
When it is about vision, not looks
With marked excess skin the fold can settle over the lashes and cut off the upper field of vision. People rarely describe this as a vision problem. They say it bothers them driving at night, that they read with their head tipped back, or that their forehead aches by the end of the day. The forehead ache comes from the frontalis muscle working constantly to lift the brow and open the view.
Once the visual field is genuinely restricted, upper eyelid surgery stops being purely aesthetic and becomes functional. That is why the surgeon examines more than the skin at the consultation: the position of the lid margin relative to the pupil, brow position, symmetry, and how completely the eye closes.
Upper lids, lower lids or both
- Upper eyelids. The incision follows the natural lid crease, so the mature scar hides in that fold. Excess skin is removed, along with a thin strip of muscle and some fat at the inner corner where needed.
- Lower eyelids, transconjunctival approach. The incision is on the inside of the lid, so there is no external scar. It is used when fat is the only problem and the skin is still good.
- Lower eyelids, incision below the lashes. Chosen when excess skin has to come out or the lid needs tightening alongside the fat pads.
- Upper and lower in one procedure. One recovery instead of two, and a lower total price than having them done separately.
What the operation involves
Blepharoplasty is normally performed under local anaesthesia. The lid area is numbed, so the procedure itself is felt as pressure and movement rather than pain. Upper eyelid surgery usually takes between forty five minutes and an hour, lower lids commonly a little longer, and a wider slot is planned when all four areas are done together.
You stay briefly for observation afterwards and go home the same day. Sutures come out on day five to seven, at a follow-up appointment. The exact plan, the approach and the number of follow-ups are agreed at the consultation, because they follow the findings rather than a general rule.

One thing surprises almost every patient: recovery is not painful, but it is tedious. Pain after blepharoplasty is rarely described as severe and usually settles with an ordinary painkiller. What lasts is the swelling, the tightness and the dry eye, and those are weeks of patience rather than days.
Risks, and what reduces them
Blepharoplasty is a well established procedure, but no operation carries zero risk and a serious consultation always says so. The most common complaint is temporary dryness and stinging, because the blink pattern changes for a while. Uneven swelling between the two eyes in the first weeks is possible, as is prolonged swelling, and more rarely a haematoma or infection. On the lower lids the surgeon pays particular attention to lid tension, because removing too much skin can pull the lid margin downwards.
- Declare everything you take. Aspirin, anti-inflammatories, omega 3 and vitamin E all increase the tendency to bleed. No medication is stopped on your own initiative, only in agreement with the doctor who prescribed it.
- Stop smoking. Tobacco slows wound healing and prolongs swelling, and quitting is the cheapest thing you can do for your own result.
- Blood pressure under control. Uncontrolled blood pressure is a frequent cause of bleeding after facial procedures.
- Take the first 48 hours seriously. Cold compresses, an elevated head and rest do more than anything you can do later.
- Say so if your eyes already water or sting. Pre-existing dry eye syndrome changes the surgical plan.
When laser can replace surgery
For mild skin laxity, fine lines and poorer texture around the eye, the fractional CO2 laser can give a very good result with no incision and no sutures. Laser works on skin quality: it stimulates collagen, tightens the surface and softens fine lines. What it cannot do is remove a true fold of excess skin or push fat pads back behind the orbital septum. For that, surgery is still the only option.
So laser is chosen when the change is early and mild, when surgery is not an option for health reasons, or when the patient simply does not want a procedure. Preparing for surgery
- Consultation. The surgeon assesses the skin, brow and lid margin position, symmetry and tear film, and only then proposes the extent of the procedure.
- Laboratory tests according to the instructions you receive at the consultation.
- Blood thinning medication is paused temporarily, strictly in agreement with the doctor who prescribed it.
- No smoking for at least two weeks before and two weeks after the procedure.
- On the day come without make-up and without contact lenses, and arrange for someone to drive you home.
